Bridging Program —
Creating Continuity of Care Before, During, and After Residential Treatment

The Gerard Academy Bridging Program utilizes community mental health resources to bridge the gap between home and residential care. This program enhances the continuity of care your child receives before, during, and after treatment. The result is a shorter length of stay and more positive outcomes for your child and family.

Individual Minnesota counties assign an independent therapist to follow your child from start to finish, meeting with your child and family on Gerard's campus and in your home.

Bridging Program


Youth Served: Boys and Girls, ages 6-18

Treatment Needs: Youth and families who can benefit from a continuity of therapeutic care from residential treatment at Gerard Academy back to their home community.

Program Model/Services

  • Increases continuity of care before, during, and after a youth is placed in residential treatment
  • Saves counties money with shorter lengths of stay and creates positive outcomes for youth and families
  • Partnership with community mental health resource, residential, and foster care services
