Family Support —

The PATH ND Family Support program provides voluntary community-based treatment utilizing the Wrap Around team process to help prevent a youth’s out-of-home placement. Specially trained, licensed PATH foster parents provide mentoring services to the family and also provide relief care for up to 6 nights a month.

Family Support


Youth Served: Families of youth, ages birth to 18, who have a mental health diagnosis and who has experienced out-of-home placement or is at risk of entering out-of-home placement.

Program Model/Services:
  • Support provided by specially trained foster parents
  • Match family’s needs and strengths with foster family support team
  • Increase the family’s ability to cope with behavioral and mental health challenges
  • Increase formal/informal supports
  • Develop workable/usable support plan
  • Support connection between child/family
  • Respond to individual and family needs
  • Mentor parents/children
  • Reduce disruptive behaviors that could lead to more-restrictive care or services
  • Provide up to six nights of relief care per month
  • Insurance-eligible: Blue Cross Blue Shield (BCBS) of North Dakota, BCBS Federal, BCBS Healthy Steps or North Dakota Medical Assistance

Goals:
  • Stabilize the child’s and family’s mental health
  • Improve behaviors
  • Provide support while increasing the child’s and family’s ability to cope with mental health challenges
  • Maintain the child in the home
